Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. As a courtesy to our patients we will file your insurance form for you, however we do not accept insurance as payment. Your fee for services rendered must be paid at the time of your visit and your insurance company will reimburse you. Visa, MasterCard and Discover are acceptable forms of payment. Care-credit financing applications are available.
Appointments
Your scheduled appointment time has been reserved specifically for you. We require 24-hours notice if you need to reschedule your appointment. We are aware that unforeseen events sometimes require missing an appointment. After multiple missed appointments without 24 hour advanced notice, you will be charged an additional fee.
